Community Care Information Management (CCIM)

The MOHLTC supports the implementation of standardized clinical assessment tools to enhance the quality of patient/client care in a number of community health sectors. The MOHLTC also decided to develop a mechanism for standardized assessments to be shared amongst Health Service Providers (HSPs) for the coordination of care and health quality improvement work at various levels. The Integrated Assessment Record (IAR) is an application that enables client information collected in assessments to be shared between the client’s circle of care across the continuum of health services in a secure and timely manner. Data in IAR is being used for reporting and research at various levels to inform service planning and quality improvement strategies. Community Care Information Management (CCIM) is responsible for the standards, clinical training and use of the Ontario Common Assessment of Need (OCAN) used in the community mental health sector. CCIM also supports the implementation and use of IAR by a number of community health sectors including mental health and the addictions sector’s staged screening and assessment tools.
